On elevated vs surface based
There's no "magic criterion", but there are two
fields on the SPC mesoanalysis page you can view:
The "effective inflow base" (color shaded part of the plot)

The height of the most-unstable parcel
http://www.spc.noaa.gov/exper/mesoanalysis/s6/mucp.gif
You can be reasonably confident that a storm is based at the ground
when the effective inflow base is at the ground (no color shading)
*and* the most unstable parcel is the surface (less than 250 m
above ground level on the most-unstable CAPE plot). A storm may
be "elevated" when the effective
inflow base reaches the ground, but the most-unstable parcel level
is above the ground.
The same information is plotted with the observed soundings on the
SPC site.

NWS will change this text to:
“THE NWS FORECASTS THE POTENTIAL FOR ____”
with the hazard type (snow, ice, wind, etc.), level of certainty, timing
and expected impact(s) clearly stated in the blank space.
For advisories
For all winter weather hazard messages that lead with the phrase
“THE NWS HAS ISSUED A(N) ______ ADVISORY”
NWS will change this text to:
“THE NWS ADVISES CAUTION FOR ____”
with the hazard type, timing and expected impact(s) clearly stated in the
in the blank space
For warnings
For all winter weather hazard messages that lead with the phrase
“THE NWS HAS ISSUED A(N)______ WARNING”
NWS will change this text to:
“THE NWS HAS ISSUED A WARNING FOR A DANGEROUS______”
with the hazard type, timing and expected impact(s) clearly stated in the
blank space
